I would include the following questions in the assessment: Have you had a similar rash before? Does it itch, burn or have any sensations? What do the borders and edges look like? Has there been any changes in the appearance of the rash? How long have you had the rash? Do you have any medication, seasonal or other allergies? Have you had any other symptoms such as appetite changes, weight loss or vomiting? Medications can also cause rashes (Burr, 2018). It is important to obtain a full list of medications. I would also ask questions about the living environment, if the patient had any signs of infestations in home, such as bed bugs. I would perform a complete skin inspection. Also, being that the patient assumed she received the rash outside, inspection of the rash to rule out Lyme disease would be very important. It is vital to recognize and start treatment of this disease as soon as possible to prevent progression of the disease and additional complications (Pearson, 2015).

Obtaining a complete medical history would be very important before deciding on treatment of the rash. Some diseases, such as autoimmune disease can present rash symptoms also. For example, Lupus can cause a major rash in some instances (Zian, Maamar, Aouni, Barakat, Naima Ghailani Nourouti, El Aouad,… Bennani Mechita, 2018).
